1. Important Notice — Not a Substitute for Therapy or Emergency Services

The Website provides general information about Affirmation for All Counseling, the services we offer, and how to contact us. The information on this Website is for informational purposes only. It is not, and should not be treated as, mental-health treatment, psychotherapy, counseling, diagnosis, medical advice, or professional advice of any kind.

No therapist-client relationship is created by visiting this Website, reading its content, sending an inquiry through the contact form, or scheduling a free consultation. A therapist-client relationship is established only after we have completed an intake process, you have signed an informed consent and other required intake documents, and we have agreed in writing to begin services.

2. Eligibility

You must be at least 18 years old to use this Website. By using the Website, you represent that you meet this age requirement. The Practice provides therapy services only to adults located in the State of Florida at the time services are received.

3. Provider Information

​

Therapy services described on this Website are provided by Nat Bartlett, a Registered Mental Health Counselor Intern in the State of Florida (Florida RMHCI License #24881), under the qualified supervision of Shana Sopko, LMHC (Florida License #MH15184). Services are provided in accordance with Chapter 491, Florida Statutes, and the rules of the Florida Board of Clinical Social Work, Marriage and Family Therapy, and Mental Health Counseling.

 

4. Use of the Website

You may use this Website only for lawful purposes and only in a way that does not infringe the rights of others or restrict their use of the Website. You agree not to:

We do not, and cannot, guarantee any specific therapeutic result, diagnosis, change in symptoms, relationship outcome, or other outcome from any service we provide or describe on this Website. Information shared on the Website about modalities such as Accelerated Resolution Therapy, group therapy, and walk-and-talk therapy describes the nature of those modalities generally and is not a promise of any specific result for you.

10. Fees and Payments

​

Current session fees, the cancellation policy, and other practice details are described on the Website and discussed during the consultation. Fees may change from time to time. The fees in effect at the time you schedule a service are the fees that will apply to that service. Affirmation for All Counseling is a private-pay practice and does not bill insurance directly. Clients are responsible for payment in full at the time of service unless other arrangements are made in writing. Our failure to enforce any right or provision of these Terms will not be considered a waiver of those rights. No waiver of any term shall be deemed a further or continuing waiver of that term or any other term.

18. Entire Agreement

These Terms, together with the Privacy Policy and the Your Rights as a Client page (and any informed-consent or intake documents you sign as a client), constitute the entire agreement between you and Affirmation for All Counseling regarding your use of the Website and supersede any prior agreements between us regarding the Website.

19. Reporting Complaints

​
