Welcome to the website of Affirmation for All Counseling. These Terms and Conditions (“Terms”) govern your access to and use of the website located at affirmationforallcounseling.org (the “Website”). The Website is owned and operated by Nat Bartlett, doing business as Affirmation for All Counseling (“we,” “us,” “our,” or “the Practice”), in Jacksonville, Florida.

Please read these Terms carefully. By accessing or using the Website, you agree to be bound by these Terms. If you do not agree, please do not use the Website.

1. Important Notice — Not a Substitute for Therapy or Emergency Services

The Website provides general information about Affirmation for All Counseling, the services we offer, and how to contact us. The information on this Website is for informational purposes only. It is not, and should not be treated as, mental-health treatment, psychotherapy, counseling, diagnosis, medical advice, or professional advice of any kind.

No therapist-client relationship is created by visiting this Website, reading its content, sending an inquiry through the contact form, or scheduling a free consultation. A therapist-client relationship is established only after we have completed an intake process, you have signed an informed consent and other required intake documents, and we have agreed in writing to begin services.

2. Eligibility

You must be at least 18 years old to use this Website. By using the Website, you represent that you meet this age requirement. The Practice provides therapy services only to adults located in the State of Florida at the time services are received.

3. Provider Information

Therapy services described on this Website are provided by Nat Bartlett, a Registered Mental Health Counselor Intern in the State of Florida (Florida RMHCI License #24881), under the qualified supervision of Shana Sopko, LMHC (Florida License #MH15184). Services are provided in accordance with Chapter 491, Florida Statutes, and the rules of the Florida Board of Clinical Social Work, Marriage and Family Therapy, and Mental Health Counseling.

 

4. Use of the Website

You may use this Website only for lawful purposes and only in a way that does not infringe the rights of others or restrict their use of the Website. You agree not to:

  • Use the Website in any way that violates federal, state, local, or international law or regulation

  • Attempt to gain unauthorized access to the Website, its servers, or any related systems or networks

  • Introduce any virus, malware, worm, or other malicious code

  • Use any automated system (including bots, scrapers, or spiders) to access or copy material from the Website without our prior written permission

  • Engage in any conduct that interferes with, disrupts, or imposes an unreasonable load on the Website’s infrastructure

"Affirmation for All Counseling," the Affirmation for All Counseling logo, and any related names, marks, and slogans are trademarks of Nat Bartlett d/b/a Affirmation for All Counseling. All other trademarks, service marks, and trade names referenced on the Website (for example, SimplePractice, Proton Mail, Spruce, Thrizer, Wix, Google, Accelerated Resolution Therapy, etc.) are the property of their respective owners.

7. Third-Party Links and Services

The Website may contain links to third-party websites, applications, or services that are not owned or controlled by us — for example, Thrizer’s out-of-network benefits eligibility checker. These links are provided for your convenience only. We do not endorse, monitor, or have any control over the content, privacy practices, or terms of any third-party website or service, and we are not responsible for them. Your use of any third-party website or service is at your own risk and is governed by that party’s own terms and privacy policies.

8. Contact Form and Communications

The contact form, email address, and any phone number listed on the Website are intended to be used for general inquiries about therapy services and to schedule a free consultation. They are not appropriate for emergencies, clinical communications, or the transmission of sensitive health information.

Email and standard text messages are not encrypted and are not considered secure means of communication. By contacting us through the Website, by email, or by text, you acknowledge and accept the inherent risks of those communication methods. We will respond using the contact information you provide. Response times vary, but we generally respond within 1–3 business days.

Submitting an inquiry does not guarantee that we will be able to take you on as a client. Availability and clinical fit are determined during the consultation process.

9. No Guarantee of Outcomes

Therapy is a collaborative process, and outcomes vary from person to person. We do not, and cannot, guarantee any specific therapeutic result, diagnosis, change in symptoms, relationship outcome, or other outcome from any service we provide or describe on this Website. Information shared on the Website about modalities such as Accelerated Resolution Therapy, group therapy, and walk-and-talk therapy describes the nature of those modalities generally and is not a promise of any specific result for you.

10. Fees and Payments

Current session fees, the cancellation policy, and other practice details are described on the Website and discussed during the consultation. Fees may change from time to time. The fees in effect at the time you schedule a service are the fees that will apply to that service. Affirmation for All Counseling is a private-pay practice and does not bill insurance directly. Clients are responsible for payment in full at the time of service unless other arrangements are made in writing. 11. Disclaimer of Warranties

THE WEBSITE AND ALL CONTENT, MATERIALS, AND INFORMATION PROVIDED ON IT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT ANY REPRESENTATIONS OR WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED. TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, IN NO EVENT SHALL AFFIRMATION FOR ALL COUNSELING, NAT BARTLETT, OR ANY OF OUR AFFILIATES, SUPERVISORS, CONTRACTORS, OR SERVICE PROVIDERS BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, EXEMPLARY, OR PUNITIVE DAMAGES — INCLUDING BUT NOT LIMITED TO LOSS OF PROFITS, DATA, GOODWILL, EMOTIONAL DISTRESS, OR OTHER INTANGIBLE LOSSES — ARISING OUT OF OR RELATING TO YOUR ACCESS TO OR USE OF, OR INABILITY TO ACCESS OR USE, THE WEBSITE OR ANY CONTENT ON IT, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.

OUR TOTAL CUMULATIVE LIABILITY ARISING OUT OF OR RELATING TO THE WEBSITE OR THESE TERMS, REGARDLESS OF THE CAUSE OR FORM OF ACTION, SHALL NOT EXCEED ONE HUNDRED U.S. DOLLARS ($100.00).

14. Modifications to the Website and Terms

We reserve the right to modify, suspend, or discontinue the Website (or any part of it) at any time, with or without notice. We also reserve the right to update these Terms at any time. The most current version will always be posted on this page with an updated “Effective Date.” Your continued use of the Website after any changes are posted constitutes your acceptance of the updated Terms. 15. Governing Law and Jurisdiction

These Terms are governed by, and shall be construed in accordance with, the laws of the State of Florida, without regard to its conflict-of-laws principles. Any dispute arising out of or related to these Terms or your use of the Website shall be brought exclusively in the state or federal courts located in Duval County, Florida, and you consent to the personal jurisdiction of those courts.

16. Severability

If any provision of these Terms is found to be unlawful, void, or unenforceable, that provision shall be deemed severable and shall not affect the validity and enforceability of the remaining provisions, which shall continue in full force and effect.

17. No Waiver

Our failure to enforce any right or provision of these Terms will not be considered a waiver of those rights. No waiver of any term shall be deemed a further or continuing waiver of that term or any other term.

18. Entire Agreement

These Terms, together with the Privacy Policy and the Your Rights as a Client page (and any informed-consent or intake documents you sign as a client), constitute the entire agreement between you and Affirmation for All Counseling regarding your use of the Website and supersede any prior agreements between us regarding the Website.

19. Reporting Complaints

If you have a concern about the professional conduct of Nat Bartlett or any therapy services provided, you may contact the qualified supervisor, Shana Sopko, LMHC, or file a complaint with the Florida Department of Health, Division of Medical Quality Assurance:
